Safety Critical Medicals: what to expect
Booked in for a safety critical medical or booking operatives in? Here’s what the assessment covers, how long certificates last, and the small preparations that stop avoidable delays on the day.
Why these medicals exist
A safety critical role is one where sudden incapacity, a blackout, hypo, or an unnoticed hearing or vision problem could endanger the worker or the people around them. Principal contractors manage that risk contractually: frameworks like Build UK’s health standards require a current fitness certificate before workers in these roles get site access. No certificate, no start. What’s tested, step by step

The whole assessment takes 30–45 minutes. With a Doctor-led provider, borderline findings are resolved on the day where possible and a certificate with conditions (wear your glasses; GP review of blood pressure) rather than a referral into a queue.
How long certificates last
Most schemes work on a 2 or 3 year cycle, shortened where age or a monitored condition warrants it. The certificate states its own expiry. For employers, the practical risk isn’t the medical itself but the lapse, a certificate that expires mid-project stands a worker down until re-examination. Recall tracking (which we include in employer programmes) closes that gap.

Individuals can book at our Stirling clinic, usually within the week. For groups of operatives, we run on-site medical days across the Central Belt and can combine the medical with statutory Health Surveillance.

Common questions
Is a safety critical medical a legal requirement?
Not directly — no single regulation names “safety critical medicals”. They’re an industry standard (led by frameworks like Build UK’s health requirements) that principal contractors write into contracts. In practice that makes them mandatory: without a current certificate, workers are refused site access. Statutory Health Surveillance duties for noise, vibration and dust apply separately and often to the same workers.
Can a worker fail a safety critical medical?
Outright fails are rare. More commonly the Doctor issues a certificate with conditions — glasses to be worn, blood pressure to be reviewed by the GP, a shorter certificate period. A finding that genuinely affects safety leads to advice on adjustments or alternative duties rather than a blanket ban.
Is drug and alcohol testing part of a safety critical medical?
Not automatically — the core medical covers health and fitness, not substances. Many principal contractors require drug and alcohol screening separately, and it can often be arranged alongside the medical in the same visit. Check what your contractor’s standard actually asks for before booking.
How should workers prepare for a safety critical medical?
Bring photo ID, glasses or hearing aids if used, a list of current medications, and details of any ongoing conditions. Avoid noise for 16 hours beforehand where possible, as it can affect the hearing test. The whole assessment takes 30–45 minutes.
