When do I need a D4 medical?
You need a D4 medical when you first apply for an HGV (Group 2) or PCV licence, when you renew at age 45 and every five years after that, and annually once you turn 65. Some employers also require one after significant illness or injury.
What should I bring to a D4 driver medical?
Bring photographic ID, your glasses or contact lenses (plus your latest optician’s prescription if you have it), a list of current medications, and details of any specialists you see. We complete the DVLA D4 form with you on the day.
How long does a driver medical take?
Around 30 minutes. The Doctor reviews your medical history, checks vision, blood pressure and general health, and completes the D4 form ready to send to DVLA.
